Rip of a specific track always fails with the same large count of 'missing' frames

Description
As mentioned on IRC, I have a specific disc here - Ryan Adams' 'Ashes & Fire' - which I can't rip successfully; track #9 always fails with 'ERROR: did not get all frames, 6036481 missing'. I've ripped a few other CDs successfully today, and it's a brand new purchase, so it shouldn't be beyond ripping. Attaching the debug log as requested.

Another case of 'me too'. For me, it's happening with 'Jars of Clay' on track 4 'Like a Child':

Apparently an encoder problem, I'm guessing. a FLAC bug?

So far, it looks to me like we have some bad CD media. I just checked some tracks again using plain cdparanoia and I'm getting a few '+' results although they are claimed as repaired.
My guess.. read length and reported length differ because of the deep error correction cdparanoia is trying, but not meeting. Too far gone to be called a clone?
I know one obvious dud I have due to a discoloration pattern will play in a cd player, but I had no idea previously the errors were so high and random for each playing. Round basket for you, duddly.

Maybe, but in my case as in others the problem has occurred with discs right out of a sealed jewel case with no visible defects. It seems unlikely that we'd run into unrecoverable defects with such frequency on new discs.

ok, looks like the last three logs I posted are media issues.
Looks like the bug for me on 'Jars of Clay' is fixed in 0.2.0 ;)

My problem with Yes - 90125 - 09 Changes is still there, but it did get through it on the 4th loop to have AccurateRip? then say the track was bad. Conclusion: I have bad media.
